There are two normal failure scenarios: either the file system on the USB drive becomes corrupt, or the USB drive stops working. There are instructions for dealing with both cases on the ripe web site, which are (simplified): corrupt filesystem: power down probe, remove USB drive, power up probe, wait 10 minutes, insert USB drive, wait 1 hour corrupt usb drive: power down probe,remove USB key and insert clean 4G+ USB key, power up probe, wait 1 hour The v3 probes seem to be less tolerant to power problems than those of the v1 and v2 probes.
