OK. Found it, it's in the *request form*. I agree that the form should demand data that the policy doesn't. (nevertheless, finding *3* ISPs that have full BGP tables shouldn't be overly hard even for a small country / small IXP - and if you are so small that you can't manage that, using a /64 from one of the members wouldn't hurt anybody - the DECIX ran on a borrowed IPv4 PA /24 for about 5 years or so, with over 50 members, and without any issues).
