Traditionally IANA has prioritized getting new ccTLDs on line once an application has been received from appropriate parties in that country.
it should be noted that defining the 2 letter ISO3166 code and delegating a ccTLD are different issues. There are two other codes without delegation (EH and KP) for more or less obvious reasons.
